  • Your original in-store or online retailer identified sales receipt dated 8/1/2020 through 8/1/2021 indicating eligible AVEENO CALM + RESTORETM product(s) purchased with purchase price(s) circled.

  • The original AVEENO CALM + RESTORETM product UPC(s) from the package(s). The UPC is the universal product code for the product. Look at the barcode to find the UPC for your product. You do not need to return the product itself.
